b. A Life Insurance policy #002 with Western Life Ins Co. with gross value of
$3,500.00 with the only beneficiary listed her deceased husband George Smith who
predeceased her (his date of death 2/1/18). Evidently she never took steps to change
the beneficiary;
c. A Raymond James brokerage account of 7,500.00 (no beneficiary declared);
d. A ½ acre vacant lot in Richmond Township gross value of $4,000.00, no lien;
After her death, her daughter Wilma Smith paid $8,005.00 for Helen’s funeral expenses out of
her own funds and can present the paid funeral bill showing she paid for the funeral. At the time
of her death Helen had two children: daughter Wilma, and son Charles. Wilma comes to your
boss I.M.Good attorney and asks for assistance in getting the only assets of Helen transferred
through Probate. She also wants to get reimbursed for her payment of the funeral expenses. You
are asked by your boss I.M Good to prepare the necessary probate papers to accomplish her goal.

the link for Forms (left side of home page), and scroll down to Small Estates to read the
Instructions. Complete PC 556. Make certain all blanks are filled in. You do not need to
fill in the file number – the file number is assigned by the court after the initial petition is
filed.

for Delivery of Certain Assets and complete that form with the appropriate information.
